Upon Arrival
You will be greeted by our friendly and courteous staff. Once you have completed the necessary patient registration forms, you will be seated promptly.
Cancellations
We request 48 hours notice of any appointment cancellations. This is not meant to be an inconvenience to you, but a way for us to use our time wisely and reschedule other patients in that time.
Payments
We are a fee-for-service practice and payment is due at the time of treatment. We offer a 10% senior discount for patients 62 years of age or more and courtesy discounts are available on larger treatment plans.

While we accept most insurance plans, we are participating providers with the following insurance companies:
